About the role

Provide individualised support to clients to identify strengths and needs and assist in setting and attaining identified goals. Work with other agencies and organisations in a collaborative approach to source appropriate support for clients.
The Afterhours Outreach Program operates within shifts 6AM to 2PM and 3PM – 11PM with outreach. A team of two working in an assertive outreach model by driving and doing foot patrols in various locations based on referrals and data provided by other stakeholders in the homelessness space; including the Red Cross Homelessness Hub, Community Patrol and the Council. The locations patrolled range from parks, the city and beaches including the strand when required. The team will offer information and referrals and assess those in particularly vulnerable situations with the possibility of putting them into emergency accommodation if eligible. You will operate from a strengths based and capacity building framework to ensure the promotion of social inclusion, human rights, participation and consumer choice.

 

What you will bring 

  • Proven ability to provide high quality, specialist casework support and case management
  • Demonstrated understanding of how to holistically assess complex client needs, including conducting risk assessments, and developing effective case plans and safety planning
  • Proven ability to identify service gaps and manage complex cases within a context of competing priorities
  • Demonstrated cultural competence and the ability to work with people from diverse backgrounds
  • Application of knowledge, skills and experience in a variety of client practice approaches and frameworks
  • Demonstrated understanding of the complex needs of children and young people is desirable
  • Current Working with Children’s Check and provisional or open Australian Drivers Licence is mandatory
