The Role
We are looking for a dedicated and passionate Part Time Practice Nurse to join our Boulder team. In this blended role, you will be a key member of the clinical team, working alongside experienced Nurses and General Practitioners to deliver high-quality care — including Chronic Disease Management (CDM) — to our local community.

You’ll be the driving force in empowering patients to better manage their health, while also supporting GPs in proactive and preventative care initiatives.

Key Responsibilities

• Prepare and review Care Plans and Team Care Arrangements
• Perform health assessments and identify chronic disease early
• Educate and coach patients on health behaviour change
• Administer vaccinations and conduct post-vaccination observations
• Triage patients and prioritise emergency situations
• Assist GPs with minor procedures and wound care
• Coordinate and maintain accurate patient clinical data
• Facilitate access to multi-disciplinary care teams
• Provide outstanding, compassionate customer service

About You

• AHPRA Registered Nurse
• Experience in General Practice and CDM
• Proven ability to conduct Health Assessments and manage chronic conditions
• Strong organisational and time management skills
• A friendly, team-oriented, and proactive approach
• Excellent communication skills and a passion for patient care
