RolePurpose
|
To carry out activities to ensure that patients and Health Care Practitioners (HCPs) are provided with high quality, safe and efficient nursing services within the medical centre.
|
Principal Accountabilities
|
|

KeyResponsibilities
|
Health Promotion and Prevention
|
- Promote and provide vaccinations to patients based on eligibility criteria from the national immunisation program schedule.
- Promote and provide vaccinations to patients in accordance with travel recommendations.
- Perform health checks and identify gaps in treatment/services where patients may benefit from additional medical services.
- Ensure patients are educated about ForHealth Medical Centres services.
- Educate patients about condition(s) and/or treatments related to their care.

Clinical Care
|
- Practice in accordance with company Standard Operating Procedures (SOPs) and/or Standing Orders; and conduct a range of direct patient care activities.
- Direct patient care activities may include: administering vaccinations and assisting with catch up schedules; administering medications; conducting ECG, spirometry and other diagnostic tests; wound care management; bandaging and plastering; removal of sutures; triage and emergency management; pre-employment medical screening and workers compensation injury management; and set up and assist with HCP-led procedures.
- Ensure accurate and timely documentation of care given.

Care Co-ordination
|
- Assist GP’s with conducting health assessments and initiate other chronic disease management services by referring to the appropriate HCP in accordance with Medicare requirements.
- Engage in the framework and support system to coordinate the care of patients.
- Ensure collaboration with clinical and non-clinical support teams to provide a holistic health care approach.
- Embed health behaviour change principles into the planning and delivery of care to empower and increase patient ownership of their health.
- Identify opportunities where patients may benefit from medical centre services.
- Monitor and support patients.

Clinical Administration
|
- Ensure clinical areas are adequately stocked and take action to replenish stock before depletion.
- Clean and sterilise instruments as required and take measures to ensure instruments are maintained and replaced when no longer in working order.
- Conduct routine activities as defined in checklists.
- Ensure clinical areas are tidy, clean and presentable.
- Complete internal training and education as requested.
- Ensure medical record documentation is accurate, timely and reflects all nursing interventions conducted and communicated.
- Support the delivery of new projects and initiatives.
- Participate in the investigation of incidents.
- Provide clinical support for accreditation

Nurse led Clinics
|
- Collect health history, plan, coordinate and document patient care.
- Evaluate patient outcomes related to patient condition(s) and/or presentations in accordance with ForHealth endorsed nursing policies, procedures, protocols and standing orders.
- Coordinate patient care and communicate patient progress and outcomes to treating GPs and escalate any adverse findings in accordance with ForHealth endorsed policies, procedures, protocols and standing orders.
- Undertake clinic administration activities (as required) to facilitate Nurse Led Clinics.

Workplace, Health & Safety
|
- Assist your manager / supervisor implement WHS policies and procedures into day-to-day operations
- Assist your manager / supervisor in ensuring workplace inspections and audits are completed
- Ensure Hazards are reported using the appropriate Hazard report tool and follow up on actions to correct any hazards identified
- Report all incidents or injuries immediately to your manager / supervisor

MinimumRequirements
|
Qualifications
|
- Current AHPRA registration as an Enrolled Nurse (EN) with medication administration education
- Diploma of Nursing
- Continuously update personal and professional skills and knowledge in order to meet the CPD requirements to maintain AHPRA registration.
|
Experience
|
- 2 years' experience providing direct clinical nursing care (desirable)
- Experience in a primary care setting (desirable)
|
CriticalSkills
|
- Knowledge of MBS item numbers (desirable).
- Understanding of health assessments, chronic diseases, prevention principles and health promotion (desirable).
- Basic computer skills
- Strong verbal and written communication skills
- Accuracy and attention to detail
