MODEL OF CARE - POSITION DESCRIPTION - ASSISTANT IN NURSING | PERSONAL CARE WORKER
POSITION PURPOSE: The key purpose of this role is to provide quality personal care and support to meet the clinical needs of residents. The Assistant in Nursing / Personal Care Worker (AIN / PCW) must follow:
- Care Managers and registered nurse instructions, and
- A daily clinical care task list.

This position reports to Opal HealthCare’s (Opal) Care Manager, or as delegated to Registered Nurse.
KEY RESPONSIBILITIES:
1. People & Leadership
- Model skills aligned with Opal Values/ Opal and Nursing policies, procedures and practices
- Participate in induction programs and ongoing training for new care team members
- Participate in annual performance/development reviews and follow the feedback provided by the GM in those appraisals
- Maintain own professional development (through performance appraisals, 360 degree feedback and resident feedback)
- Ensure daily resident care problems that arise are reported immediately to avoid disruption to care to residents
- Participate honestly/positively in client and team members surveys
- Maintain professional knowledge and skills by participation in mandatory training
- Work effectively as a member of the clinical care team
- Provide appropriate support in the orientation of new team members including acting in the role
of “buddy” to new team members as delegated
2. Quality & Care
- Provide high standard services that support and assist residents to achieve maximum independence by meeting daily living and personal requirements within the plan of care, and with consideration of individual preferences
- Under the direction of the Registered Nurse and as appropriate to skill set, assist with implementing the residents plan of care related to medication administration
- Following appropriate reporting mechanisms to meet duty of care requirements
- Report all changes in health status to the Registered Nurse or their delegate
- Direct all enquiries regarding plan of care to the Registered Nurse or their delegate
- Follow WHS standards/safe work practices and reporting expectations
- Follow Service Level Agreements (SLAs)
- Support and assist in the provision of a clean, comfortable, safe and secure environment
- Actively support continuous improvement initiatives
- Identify any opportunities for service improvement and advise the Care Manager and/or General Manager as appropriate
- Maintain residents sensitive and confidential data/information, ensuring to maintain confidentiality about
- business or operational interests

3. Marketing
- Maintain aged care industry knowledge to ensure clinical care
- Act as a Opal ambassador and promote Opal with team members and community
- Develop and maintain positive resident/family and team relationships
4. Financial
- Participate in clinical roster with no dropped shifts to maintain care and reduce costs
- Seek approval from team leader/manager (as appropriate) prior to incurring any expense

- Certificate III in Community Services (Aged Care)
- Knowledge of the aged care industry and regulations
- Current police check (within last 3 months)

POSITION PURPOSE: The key purpose of this role is to provide person centred care and support to enable residents to live a meaningful life. Under the direction of the Registered Nurse, Cert IV AINs/PCWs take a person centred approach to resident wellbeing, encompassing the bio-psycho-social needs of residents. Their attention to detail and commitment to personal touches creates and maintains environments that are homelike and conducive to physical, social and emotional wellbeing of residents, visitors and team. Team work within a competency based framework and their scope of practice.
This position reports to Opal HealthCare’s (Opal) Care Manager, or as delegated to the Registered Nurse.
KEY RESPONSIBILITIES:
1. The Resident Experience
- Know residents deeply, including their preferences, likes, dislikes and routines of life
- Enhance resident health outcomes through dignified daily living routines and direct care activities
- Share a commitment to providing residents with enjoyable food experiences by assisting residents with their meals and snacks.
- Ensure residents remain well hydrated
- Partner with residents to support continued independence and retain mobility by assisting with walking, use of wheelchairs and other aids to enable residents to continue to participate in their lives
- Maximise the resident’s physical function by assisting with physio-reablement and offering activities that promote strength and mobility.
- Support and maintain a clean, comfortable, safe and secure home environment.
- Respectfully handle resident clothing and personal items
- Maintain clean and tidy environments including resident rooms and common areas.
- Provide supportive sleep environments with fresh linen, towels and well-made beds

3. The Opal Way

- Under the direction of the Registered Nurse, and within the scope of practice, assist with implementing the residents’ agreed plan of care related to medication management.
- Complete the required clinical documentation in line with Opal HealthCare’s procedures. This may include progress notes, charting and contributing to resident care as directed by the Registered Nurse.
- Under the direction of the Registered Nurse, and within the scope of practice, assist with completing basic observations and clinical care (prophylactic dressing application).
- Complete the required clinical documentation in line with Opal HealthCare’s procedures. This may include progress notes, charting and contributing to resident care as directed by the Registered Nurse.
- Direct all enquiries regarding the agreed plan of care to the Registered Nurse.
- Report all changes in a resident’s health status to the Registered Nurse.
- Identify and escalate hazards and incidents to the Registered Nurse and contribute to the incident investigation process.
- Follow WHS standards/safe work practices and reporting expectations.
- Recognise and respond to the changing needs of infection prevention and control (see policies and procedures).
- Assist and support in the management of emergencies.
- Adhere to Opal HealthCare’s policies and procedures.

EXPERIENCE/QUALIFICATIONS:
- Certificate IV in Individual Support, Ageing Support, Disability Services or Community Services
- HLTHPS007 ‘Administer and monitor medications in the Certificate IV in Aged Support’ (Or working towards)
- Knowledge of the aged care industry and regulations
- Current police check (within last 3 months)
- NDIS Worker Screening
