About the program :
The “Continuing to be Me” C2bMe@Home program is a comprehensive in reach person centred model of care designed to enable older persons living in home requiring mental health support. It is designed to reinforce identity and purpose, increase connectedness, develop hope, reduce symptomatology, and optimise function so they can continue to live with dignity. C2bMe @Home offers low and medium intensity interventions through therapeutic interventions delivered by skilled mental health clinicians that are tailored to the older person's needs.
About the Role
Clinician role involves the provision of comprehensive, evidence-based clinical care coordination, psychological and psychosocial interventions consistent with the model of care. The Clinician will be responsible for individual therapy, liaising with GP and other stakeholder. They will support consumers in their psychological recovery from isolation aiming to optimise functioning of consumers to continue living with dignity.
About you in the role
- Work as a part of the multidisciplinary team to ensure participants are identified and have access to the services in a timely manner
- Provide comprehensive and evidence based therapeutic interventions including in home functional recovery activities in line with the Service Delivery Principles of C2bMe
- Plan, manage and implement care plans in collaboration with the consumer, carer, and other members of the integrated care team including GPs
- To deliver capacity building and education sessions to family and carers
- To be innovative in clinical service provision, and to contribute to current evidence-based practice.
What you need to succeed
- Qualifications in Social Work, Psychology, Nursing (RN), Occupational Therapy, or Counselling.
- Registration with relevant professional bodies (APA, PACFA, or AASW).
- 1–2 years’ experience in mental health intervention.
- A valid driver’s license and own vehicle.
- Familiarity with electronic medical records (EMR’s)
- Ability to work autonomously and stay organised
